STEP

1

Obtain your MMCC
Patient ID Number

Obtaining your 16-digit MMCC Patient ID Number prior to your evaluation visit means that you can go straight to the dispensary on the same day right after your evaluation. You can register for your Patient ID Number by following this link: https://mmcc.health.maryland.gov/Registry/RegisterPatientCG.aspx?id=2.

In order to register, you will need to have:

  • a qualifying condition (see below)

  • a government-issued identification card (such as a driver’s license or passport)

  • a valid email address

  • a passport size photo (from the shoulders up with a white background; no hats or glasses).

***You do not need an appointment to get your ID number and can apply directly on the MMCC website. If you need assistance with registering for your MMCC Patient ID Number, you can schedule an appointment and our staff can assist you at that time. After successfully registering on the MMCC website, you will receive a 16-digit Patient ID Number via email in about 2-4 weeks. You will be required to return to the office with your Patient ID Number / schedule a second telemedicine visit before obtaining your written certification from our office.

2

Gather Medical “Proof” of a Qualifying Condition

We are currently able to prescribe THC-based products for any of the approved conditions listed on the Maryland Medical Cannabis Commission Website. These include: cachexia, anorexia, wasting syndrome, severe or chronic pain, severe nausea, seizures, severe or persistent muscle spasms, glaucoma, and post-traumatic stress disorder (PTSD). We can also prescribe for mental health concerns such as panic attacks, chronic anxiety, depression, and suicidal ideation.

 

For your first visit with Arise Alternative Medicine, you will need to provide any documentation that can be used to verify your medical condition. This includes things such as: pill bottles, prescriptions, letter from a medical provider and/or anything else that confirms your medical history with the condition.

Please note, it is strongly encouraged to provide your medical records prior to your evaluation. If you do not provide your medical records, there will be a $25 fee for medical record retrieval.

Evaluation

We offer both in-person and telemedicine medical marijuana evaluations. During our visit, we can provide:

  • Advice on how to get your 16-digit MMCC Patient ID Number

  • Medical record retrieval, as needed

  • Confidential medical record reviews and evaluations

  • Drug interaction considerations

  • Same-day medical cannabis certifications

  • Recommendations about which medical marijuana products and dosing are best for your specific needs
